ITEMS TO BRING WITH YOU
1. Application Form
2. Certified Birth Certificate from the State you were born. (Hospital Birth Certificate is not accepted)
3. Two (2) identical Passport Pictures
    Must be 2x2’s with light background. (No lettering or photos on
    clothing, no eyeglasses)
4. State issued ID: Driver License or ID Card. (may use Military ID)

EXPEDITE SERVICE
You may expedite your application to receive your passport between 2 and 3 weeks (regular 6-8 weeks). The Fee is an additional $60.00 for a total of $190.00 (Adult) or $160.00 (Children) Made Payable To U.S. Department of State, plus $35.00 in cash (The Passport Card can not be Expedited.)

OPTIONAL SERVICE - FOR PASSPORT BOOK ONLY
Overnight Return Delivery Service Fee: $23.36
If you are using this fee, please include this fee in you check. 

NOTE:
1. ALL CHILDREN UNDER 16 YEARS OF AGE MUST HAVE BOTH
    PARENTS CONSENT FOR A PASSPORT.
2. ALL PERSONS, ADULT OR CHILDREN, MUST BE PRESENT AT
     TIME OF APPLYING FOR A PASSPORT.
3. Passports are good for (10) years. If you are under age 16, Passport is good
     for (5) years.

FEES REQUIRED for Passport Card

This document may not be used for air travel. It will only allow re-entry at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda.
