How to Use the Application

First Download the application by clicking on the image below

Instructions

1. Open the document in a PDF viewer.
2. Fill out the document with all information that you can
3. Save the file with all of your edits.
4. Create an image for the section on page 3 of the application that asks you to provide a sketch and location of proposed work. (This can be done by drawing on paper and taking a clear photo with a phone, drawing on a map and saving a screenshot, etc.)
5. Get a digital copy of your "Traffic Control Plan"  (this can be an image, pdf, etc.)
6. Come back to this page and fill out the form below. In doing so you will be asked to upload 3 items:
   •  the PDF File you just filled out and saved (ex. "application.pdf")
   • an image file to represent your "Sketch and Location of Proposed Work" asked for on page 3 of the application (ex. "image.jpg")
   • a Traffic Control Plan
7. Click the green "submit" button at the bottom of the form and you are done.
